How to file a claim
 
Minimum Wage and Overtime/Living Wage: claims can be taken over the telephone by a Compliance Specialist; or the employee may visit the office.
 
Wage Payment/Sick and Safe Leave cases:  by law, employees must provide a written request and a notarized assignment for the Wage-Hour office to handle a claim and collect any wages found due on their behalf.  If an individual cannot visit the office, a form is mailed to the employee along with the assignment form.  The individual is asked to return along with the notarized assignment.
